TARGET AUDIENCE
This educational activity is intended for healthcare providers (HCPs) who treat patients with type 2 diabetes (T2D) in the primary care setting. Specifically, members of the primary care team, including physicians, nurse practitioners (NPs), PAs, and other members of the health care team who treat patients with T2D.

PROGRAM OVERVIEW
Primary care providers are in a unique position to address both diabetes and weight management in the same patient at the same time, partially through judicious choice of medication that has beneficial effects on both HbA1c and weight. Three short patient case modules address attitudes and beliefs that may be barriers to optimal weight management for patients with T2D.

LEARNING OBJECTIVES
Through the application of the strategies, tools, and resources shared in this education, learners will demonstrate improvements in their confidence, knowledge, and skills to:

• Identify common attitudes and beliefs that are barriers to optimal weight management for patients with T2D
• Utilize current research and guideline recommendations to modify treatment plans to optimize weight management for patients with T2D
• Integrate strategies for weight-management planning into shared decision-making discussions with patients with T2D

There are no fees for participating and receiving credit for this activity. Successful completion is achieved by reviewing the CE information, completing the pre-test, completing the activity, and completing the post-test and evaluation form. A score of at least 75% on the posttest is required to successfully complete this activity. The participant may take the test until successfully passed.
