TARGET AUDIENCE
This education is intended for endocrine specialists including endocrinologists, advanced practice professionals (dietitians, nurse practitioners and physician assistants), nurse and diabetes care and education specialists, and primary care providers (MDs, PAs, nurses) who care for patients with diabetes.

STATEMENT OF NEED
The overarching goal of this initiative is to improve understanding of glucose monitoring, report interpretations, and the use of this information to optimize therapeutic management, leading to practice transformation and increased shared decision-making. The planned educational and awareness programs can help participants implement systemic health system changes to improve overall patient outcomes.

ACCREDITATION STATEMENT
The American College of Endocrinology is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

CREDIT DESIGNATION
Physicians: The American College of Endocrinology designates this enduring activity for a maximum of 0.5 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Maintenance of Certification: Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 0.5 MOC point(s) in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC point(s) equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit. Aggregate participant data will be shared with commercial supporters of this activity.

American Nurses Credentialing Center (ANCC)
Association of Diabetes Care & Education Specialists designates this activity for a maximum of 0.5 ANCC contact hours.

The Association of Diabetes Care & Education Specialists is approved by the California Board of Registered Nursing, Provider Number 10977, for 0.5 contact hours. RNs must retain this document for 4 years after the activity concludes.

RNs should only claim credit commensurate with the extent of their participation.

Commission on Dietetic Registration (CDR): CDR Credentialed Practitioners will receive up to 0.5 Continuing Professional Education units (CPEUs) for completion of this activity. Completion of this RD/DTR profession-specific or IPCE activity awards CPEUs (One IPCE credit = One CPEU). RDs and DTRs are to select activity type 102 in their Activity Log. Performance Indicator selection is at the learner’s discretion.

Certified Diabetes Care and Education Specialists: To satisfy the requirements for renewal of certification for the Certification Board for Diabetes Care and Education (CBDCE), continuing education activities must be diabetes related and approved by a provider on the CBDCE list of Approved Providers (www.ncbde.org). CBDCE does not approve continuing education. The Association of Diabetes Care & Education Specialists is on the CBDCE list of Approved Providers.
